{"id":5946,"date":"2021-08-26T17:39:02","date_gmt":"2021-08-26T17:39:02","guid":{"rendered":"https:\/\/www.skincancer.org\/?p=5946"},"modified":"2024-02-07T19:26:32","modified_gmt":"2024-02-07T19:26:32","slug":"ask-the-expert-what-do-you-say-to-a-client","status":"publish","type":"post","link":"https:\/\/www.skincancer.org\/blog\/ask-the-expert-what-do-you-say-to-a-client\/","title":{"rendered":"Ask the Expert: What Do You Say to a Client?","gt_translate_keys":[{"key":"rendered","format":"text"}]},"content":{"rendered":"<h4>Q: As a massage therapist, I see a lot of skin, and sometimes I\u2019ll notice spots or sores on my regular clients that look different or don\u2019t seem to be healing. I\u2019m not an expert, but I feel like I should say something. What do you suggest?<\/h4>\n<p>You\u2019re right that massage therapists are well situated by virtue of the work they do to notice a lesion or other suspicious-looking spot on a client\u2019s skin. So are <a href=\"https:\/\/www.skincancer.org\/blog\/a-haircut-could-save-your-life\/\">hairdressers<\/a>, cosmetologists, spray tan artists and aestheticians. But as you also note, pointing it out can be tricky because you lack expertise in these matters.<\/p>\n<p>In a current study I\u2019m doing with massage therapists, which involves training them in lesion detection, we discovered that an overwhelming majority of them, whether licensed or not, had almost no familiarity with <a href=\"https:\/\/www.skincancer.org\/skin-cancer-information\/\">skin cancer<\/a>. (Licensing varies considerably from state to state. Some states require a license to practice massage therapy, but others do not, and there is no standardization of training programs nationally or even within states.) So it\u2019s understandable that you would hesitate to broach a subject about which you feel uncertain yourself. Still, your impulse that if you \u201csee something, say something\u201d is unequivocally the right one. The issue is <em>how<\/em> to say it. Obviously, someone getting a massage is there to relax and de-stress, so the goal is to get the client\u2019s attention without creating fear or anxiety.<\/p>\n<p>In most cases, it\u2019s better to wait until the massage is over to have the conversation. Suppose a regular client comes in with <a href=\"https:\/\/www.skincancer.org\/risk-factors\/atypical-moles\/\">an \u201cugly duckling\u201d mole<\/a> [i.e., one that stands out from the ones surrounding it] on their lower back that you don\u2019t remember seeing before or that looks slightly different. You might say, in a neutral, supportive tone, \u201cThere\u2019s a spot on your lower back I haven\u2019t noticed before \u2014 have you?\u201d<\/p>\n<p>Your client will probably say, \u201cNo,\u201d because the lower back is not a place most of us look at very often. You could say, \u201cI don\u2019t recall seeing it before, and it\u2019s shaped a little funny. Would you like me to take a photo with your phone?\u201d If the client agrees and then looks at the photo and says, \u201cYeah, that is weird \u2014 what do you think it is?\u201d don\u2019t reply, \u201cIt could be skin cancer.\u201d Rendering an amateur diagnosis is exactly what you <em>shouldn\u2019t<\/em> do. Nor should you get personal, saying something like, \u201cOh, I had the same thing and it totally freaked me out! But it turned out to be nothing.\u201d Instead, emphasize that you\u2019re not qualified to make medical judgments but do strongly believe that clients should be aware of spots. And then, ideally, you can help further by providing your client with a list of local dermatologists (though you should refrain from giving a referral to a specific one) or perhaps a <a href=\"https:\/\/store.skincancer.org\/brochures\/\">brochure <\/a>from an organization such as The Skin Cancer Foundation.<\/p>\n<p>Realize that you may get a bit of pushback. Perhaps you have a client with a great many moles, which you happen to know is a risk factor for melanoma. You could say something like, \u201cI see a lot of moles on your back. Would you like me to avoid them during the massage?\u201d If your client asks, \u201cWhy do you care?\u201d don\u2019t get defensive. Simply say, \u201cI like to keep an eye on my clients\u2019 skin while I\u2019m massaging them. I\u2019m not a dermatologist, but it\u2019s important to know your moles.\u201d And again, offer to take a photo with the client\u2019s phone and then steer them to additional resources. In my dreams, every spa in the country would have charts on the walls showing the <a href=\"https:\/\/www.skincancer.org\/skin-cancer-information\/melanoma\/melanoma-warning-signs-and-images\/#abcde\">ABCDEs of melanoma<\/a> and would make brochures and lists of local dermatologists available to every client. Unfortunately, we\u2019re not there yet.<\/p>\n<p>In short, your role is not to diagnose or direct, but to guide \u2014 calmly and without judgment.
